I haven't tried realistic damage as of yet but I might soon because I found it strange that in the base game (or rather, with Warfighter), I landed like 5 or 10 shots with a shotgun on a bandit and he didn't go down and yet, with his sawn off shotgun, he managed to inflict heavy damage on me at medium range on master. It simulates breathing into it as well and provides animations when harvesting mutant parts. You have a mask overlay when wearing a mask and it changes for every damage it takes. It's an addon that greatly enhances realism, in my opinion. I recommend to install it after getting the files for your chosen weapon addon and outfit addon installed in CoC.
